2009 Application Materials
To be eligible for consideration, nominees must be 35 years of age or younger on Friday April 17, 2009, and must have a minimum of three years of professional experience working in the area for which they are being nominated. Nominators are encouraged to think creatively about the myriad methods by which up-and-coming professionals can contribute to a Chicago community where race and class barriers no longer limit people’s full participation in all aspects of society.

Nomination Procedures
1) Individuals may nominate as many people as they like. You may nominate yourself. (If you choose to nominate yourself, have a professional reference fill out the Nomination/Recommendation form)
2) Nominations are due to the Community Renewal Society on Friday April 17, 2009, and should include:
• A Nomination Cover Sheet
• A Completed Nomination/Recommendation form
• A Completed Statement from the Nominee
• A Brief Bio of the Nominee (2 paragraphs max.)
• Optional additional attachments such as:
Resume/CV, press clips, examples of impact, creative materials, etc.
Submission Procedures:
Complete applications are due to the Community Renewal Society on Friday, April 17, 2009.
